For most transgender men, FTM top surgery is likely the most meaningful milestone in their gender affirmation journey. While the procedure removes unwanted breasts and breast tissue and creates a masculine chest contour, the ultimate goal for most patients is not to achieve a bodybuilder’s physique. Instead, it is to feel comfortable, confident and at home in their own body.
For some individuals, however, the journey doesn’t stop after surgery. They choose to start or continue building strength and muscle, creating a desired athletic appearing chest. Some of these patients can produce remarkable results that demonstrate what is possible when excellent surgery is combined with commitment, patience and healthy living.
It is important to understand, however, that these outcomes represent one end of a broad spectrum. They are inspiring examples—not expectations.
Surgery Creates the Foundation
Double-incision FTM top surgery removes excess breast tissue and skin while repositioning and resizing the nipple areola complexes to create a masculine appearance. During surgery, the remaining tissues are carefully sculpted to produce natural contours that complement each patient’s anatomy.
Although surgery establishes the framework, it does not create or even directly affect muscle. The appearance of a well-defined chest comes from development of the pectoralis major muscles through strength training and healthy lifestyle habits. Patients who have built these muscles before surgery often notice that their muscular definition becomes much more apparent once the breast tissue has been removed.
The Difference Muscle Can Make
Patients who enjoy resistance training frequently ask whether building chest muscles before surgery is worthwhile.
The answer is often yes.
Well-developed pectoral muscles can enhance chest definition, create a broader masculine appearance, and improve the transition between the chest, shoulders and upper abdomen. While surgery cannot create these muscles, it allows them to become visible in a way that typically wasn’t possible beforehand.
For patients who continue strength training after healing is complete, chest definition may continue to improve over time.
Setting Realistic Expectations
Photographs of fitness influencers, bodybuilders and professional athletes are everywhere. While these physiques can certainly be motivating, they can also create unrealistic expectations.
The reality is that very few people—transgender or cisgender—naturally develop an exceptionally muscular chest.
Even among cisgender men, achieving a highly developed chest usually requires years of consistent resistance training, excellent nutrition, sufficient protein intake, proper recovery and favorable genetics. Some individuals work incredibly hard yet never develop the dramatic muscle size seen in magazines or online because genetics largely determine muscle shape, muscle insertions and overall body composition.
The same is true after FTM top surgery.
Every patient begins with different anatomy, different genetics, different athletic abilities and different goals. Success should never be measured by comparing yourself to someone else. Instead, success is achieving the result that allows you to feel comfortable and confident in your own body.
Every Patient Is Different
No two patients will have exactly the same result.
Chest width, rib cage shape, skin quality, skin elasticity, body fat, existing muscle mass, genetics, and individual healing characteristics can all influence the final appearance.
This is why an outcome that looks excellent on one patient may look very different on another. The goal of top surgery should always be to create a masculine chest that is appropriate for the individual patient’s body and personal goals.
Confidence Is the Greatest Transformation
While photographs often focus on physical appearance, they rarely capture the emotional changes that occur after top surgery.
Many patients describe finally feeling comfortable wearing fitted clothing, exercising in public, swimming, going to the beach or simply looking in the mirror without the discomfort they once experienced. That newfound confidence is often the most meaningful outcome of surgery.
For patients who enjoy fitness, top surgery also removes a barrier that may have prevented them from fully appreciating the results of hard work in the gym.
The physical transformation can be impressive.
The emotional transformation is often even greater.
An Inspiring Example
The patient featured in the accompanying photographs represents just one example of what may be possible. His outcome is remarkable but it should not be interpreted as the expected result for every patient. Rather, it reflects the combination of carefully performed surgery, individual anatomy, consistent strength training, disciplined nutrition and unwavering personal commitment.
Perhaps most importantly, it reflects confidence after gender affirming top surgery.
The photographs demonstrate much more than muscular development. They illustrate the freedom to feel comfortable in one’s own body and the confidence that can come from aligning physical appearance with gender identity.
This type of result is not the primary goal for most individuals seeking FTM top surgery, nor does it need to be. The overwhelming majority of FTM patients simply want a masculine chest that allows them to live more comfortably and authentically. That outcome is every bit as meaningful.
One FTM Patient’s Body Building Results
This 18-year-old transman came out four and a half years ago and had been living this way ever since. He had been consistently binding with tape. Testosterone had been taken for slightly less than eight months (photos A, C and E). It was his desire to have a flat masculinized chest with nipple-areola complexes. He did not work out nor share a desire to have a well sculpted chest.
His gender affirming FTM top surgery was performed using the double incision free nipple graft technique. The postoperative course was unremarkable and he was compliant with instructions and guidelines including the usage of silicone tape for scar treatment.
The results from his surgery exceeded his expectations (photos B, D and F). It led to numerous positive impacts on his life – physically and emotionally. In fact, he was motivated to start working out and developing a more muscular appearing chest. Results are seen at one year after surgery.
