Clearly a gym rat.
Well, first of all – if you want your chest to look like this you must be truly dedicated and work out hard several days a week.
You must also lay off the carbs and other things that are bad for you. Instead, you have to follow a healthy, sensible diet.
A low or normal BMI (body mass index) is a must in order to obtain this look. Being overweight or obese – forget about it!
And if you were AFAB, are now are a transman and would love to have a chest like this – what are you thinking? Seriously!
Or better yet: What are you smoking?
This is an impossibility.
Or is it?
In the interest of full disclosure, this muscular dude didn’t start out as one. Or either one – to be more precise.
That is, he grew up thin and just average in appearance. Nothing special.
And, he started out as a girl.
Yes!!
This is absolutely true.
His Story
This 20-year-old FTM patient of mine knew for years that he was not really a girl. Five years previously he came out as a transman and had been on testosterone for four years. He worked out and tried to develop a more masculine appearing chest. Now, he was ready for gender affirming FTM top surgery.
During his consultation with me, he was noted to have a BMI of 23 which is in the normal weight range. His breasts were around a “B” cup size (photos A, C and E). In a natal male, this looks just like routine gynecomastia.

The Procedure
The ideal FTM top surgery procedure for him was the concentric circle (periareolar) approach. This allows for the removal of breast tissue, reduction in size of the nipple-areolar complex and skin tightening. He agreed and the surgery was performed without any problems.
He did very well during the postoperative course and had no problems. Subsequently, he resumed working out and developed an even more muscular and defined masculine chest. Results are seen at one year (photos B, D and F).
As expected, he is extremely happy with the outcome with the results being life changing.
Having Realistic Expectations for Your Results
These results shown here are not the typical ones seen with FTM or FTN gender affirming top surgery. So, if you really like and want results like this, it would be highly unlikely that you can have a similar outcome. Possible but not highly probable.

To start with, you would need a similar situation including the anatomy, small breast size and a low or normal BMI. Then, dedication to working out including as relates to the chest is a must.
