Breast augmentation has long been one of the most popular cosmetic procedures in the United States. Every year, hundreds of thousands of women choose it to enhance their figure, restore volume after pregnancy or weight loss or achieve improved symmetry.
While the performance of the procedure by a plastic surgeon is of utmost importance, the planning process, especially selecting the right breast implant, is also extremely important in shaping your final result.
The “ideal” implant isn’t simply about size. It also involves profile, fill material (silicone versus saline), feel and placement – all chosen to match your proportions, lifestyle, personal vision and other factors.
In this guide below, I share with you my advice based on over 38 years of extensive experiences with breast augmentation and breast implants so as to help you navigate your choices and find the perfect fit for you.
1. Begin with Your Goals
Think carefully about what you hope to achieve. Do you want a subtle, natural-looking enhancement? More pronounced and glamorous curves? Volume restoration after breastfeeding? Or correction of asymmetry?
Being honest about your goals is essential. Bring reference photos if you can – they’re a helpful visual starting point. Your preferences are the most important factor in shaping your final outcome. Cup size and relative proportionality can help though are somewhat subjective and imprecise by nature.
2. Remember That Size Isn’t Everything
Breast implants are measured in cubic centimeters (ccs), not cup sizes, so one doesn’t select a “C” or “D” cup size implant. Furthermore, the same volume can look very different on different body types and pre-existing breast sizes.
Beyond volume, implants vary in profile (low, moderate, high and ultra-high), fill material (silicone and saline) and placement (above or below the muscle). Each factor can have a significant impact on your overall look.
3. Choose an Implant That Fits Your Frame
Implants should complement, not overpower, your natural proportions unless that is the look that you are seeking. Selecting an implant that’s too large for your frame can cause an unbalanced look and strain your tissues over time which can lead to earlier and more significant drooping. Going too small may leave you wishing for a more noticeable change.
As part of the planning for your surgery, I use precise chest and breast measurements to further assist in identifying implants that may be more harmonious and appropriate for your goals.
4. Experience Your Options before Deciding
At your consultation, you’ll likely have the chance to try on sizers – sample implants that can be placed in a bra to simulate what you may be able to expect from surgery.
5. Match Your Choice to Your Lifestyle
If you’re highly active or regularly engage in sports, a moderate-sized implant may be more comfortable and fit your active lifestyle better. If you enjoy dressing for events or prefer a curvier and more provocative silhouette, a fuller implant with more projection and volume might be more appropriate for you.
Your surgeon will also explain how implant types and placement may affect your comfort during physical activities and in daily life.
6. Think Long-Term
Breast implants can last for many years, but they are not lifetime devices. Life changes such as pregnancy, weight fluctuations or aging can influence your breast appearance, and you may eventually want a revision.
Larger implants may place more strain on the skin and tissue, potentially leading to earlier sagging and the need/desire for revision surgery sooner. Choosing a proportionate size often results in a more enduring, natural look.
7. Give Yourself Time
Avoid rushing into a decision. Schedule your consultation well in advance of any important events and take time afterward to reflect.
Well-meaning friends and family may offer opinions, but remember: this is about what makes you feel confident and happy. You should aim to please yourself – not others.

📌 Quick Guide: Fast Facts About Choosing Breast Implants
- Saline vs. Silicone: Silicone often feels more natural; saline offers adjustable volume and easier leak detection.
- Profiles Matter: Higher profiles project more but have a narrower base; moderate profiles balance projection with width.
- Cup Sizes Are Misleading: Bra sizing is inconsistent—implant volume and profile give a truer idea of your result.
- Lifestyle Counts: Active women often prefer more moderate sizes for comfort and mobility.
